I acknowledge
- that Coronavirus/COVID-19, and its potential variances and mutations, are highly contagious and that – in the opinion of some public health commentators, notwithstanding various advisories from government authorities
- that Renaissance Weekend – its management, staff, independent contractors, and supporting entities – have put in place preventative measures to reduce the spread of such infectious viruses, but cannot and do not guarantee that I, or my family members and/or associates, will not become infected with such viruses;
- the risk of becoming exposed to and/or infected by such viruses may result from the actions, omissions or negligence of myself or others, including, but not limited to, Renaissance Weekend’s organizational team, my fellow Renaissance Weekend participants, their families and guests, the hotel/resort’s staff, service-providers and other guests;
- I voluntarily am participating in Renaissance Weekend – which, like other public gatherings, may increase my risk to exposure to such viruses – and agree to comply with all Renaissance Weekend policies and procedures to reduce the potential spread of such viruses.
I attest that I
- have received and completed one of the U.S. Food & Drug Administration-approved vaccination regimens (including subsequent booster doses) and am not experiencing any symptom of illness (such as cough, shortness of breath or difficulty breathing, fever, chills, muscle pain, headache, sore throat, or new loss of taste or smell);
- have not recently been diagnosed with such viruses;
- do not believe that I have been exposed to someone with a suspected and/or confirmed case of such virus;
- am following CDC-recommended guidelines as much as possible and have been limiting my exposure to such viruses.
I agree that
- If I, or any member of my party, tests positive and/or experience(s) any symptoms of such viruses during the Weekend, I/he/she shall immediately notify a Renaissance Weekend staff member, will immediately discontinue such participation in the Weekend, and will seek appropriate medical attention.
I hereby release and agree to hold Renaissance Weekend, its management, staff, independent contractors, and supporting entities, the owner, management and staff of the host property, and all past, present and future affiliates, successors, assigns, employees, volunteers, vendors, partners, directors, and officer of such entities, as well as my fellow Renaissance Weekend participants, harmless from, and waive on behalf of myself, my heirs and my personal representatives any and all causes of action, claims, demands, damages, costs, expenses and/or compensation for damages or loss to myself and/or property that may be caused by any act, or failure to act of Renaissance Weekend and the aforementioned parties, or that may otherwise arise in any way in connection with my participation in Renaissance Weekend.
I understand that this release discharges Renaissance Weekend, its management, staff, independent contractors, supporting entities and their officers and advisers, all the aforementioned parties, and my fellow Renaissance Weekend participants from any liability or claim that I, my heirs, or any personal representatives may have against them with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection, to, my participation in Renaissance Weekend, including but not limited to first aid, treatment, or service rendered in connection with my participation in the Weekend.
By attending and/or participating in Renaissance Weekend and execution of this Agreement, I have given a full release of liability to the released parties to the full extent permitted by law.
